This is the part that gets left out. Every person alive surfaces briefly between sleep cycles, all night long, adults included. You do it and resettle so fast you do not remember. Your baby is doing the same thing. So the goal is not to stop the surfacing, because you cannot, and you would not want to. The goal is what happens next.

what decides whether it becomes a full wake-up

If your baby falls asleep at bedtime being fed, rocked, or held, that becomes the thing they associate with falling asleep. So when they surface at 1am, which they will, they look for that same condition to get back down, and they cannot recreate it alone. That is the wake-up you are getting called into. It is not hunger and it is not habit in the bad sense, it is a learned starting condition, and it is completely undoable.

why the fix is at bedtime

Because the association is formed at the start of sleep, that is where you change it. If your baby can practice falling asleep at bedtime with a little less help, the middle-of-the-night surfacings stop turning into full wake-ups on their own, because there is nothing to go looking for. How gently you make that shift depends entirely on your baby and on what you can hold to consistently, which is the piece I build with each family rather than hand out.
